SPA Clinical Information


The Single Point of Access (SPA) is a result of partnership working between NHS Corby and Nene Clinical Commissioning Groups, Kettering and Northampton General Hospitals, Northamptonshire Healthcare NHS Trust, Northamptonshire County Council and Olympus Care Services (Northamptonshire Adult Social Services –NASS) and Voluntary Services.

The purpose of the SPA is to ensure that patients/clients are given the right care, at the right time, every time. Delivering solutions that allow people of Northamptonshire, to live at home for as long as possible, maximising independence.

With the overall objective of the SPA to improve the flow of patients requiring support on discharge out of the urgent care system, significantly reducing the number of patients experiencing a delayed transfer of care. 

It ensures that changes support safe, effective and timely delivery of quality care with positive patient outcomes and equality of access and outcomes for all patient groups. 

The SPA obtains information on the patients that fall into the complex discharge category, by way of a telephone referral and the completion of a Patient Discharge Needs Assessment (PDNA), the patient does not need to be clinically stable.

The SPA will have oversight and strong links with all areas of the health and social care sectors and will endeavour where practicable to utilise alternative services in a co-ordinated fashion to maintain people in their own homes where possible.
